數位人道協會獲 Google.org 百萬美元資助 啟動「數位健康普惠計畫」以科技彌補衛教落差,實現城鄉照護公平性與社群醫療韌性

數位照護新模式:填補城鄉健康落差

在偏鄉與社區現場,長者普遍缺乏併發症風險意識。數位人道協會指出:要消弭健康不平等,科技必須真正走進社區。我們致力於支持第一線進行照護指引與行為改變,從根本改善慢性病的惡化趨勢。作為此倡議的關鍵夥伴,數位人道協會肩負著彌補病患衛教與參與落差的重任,確保科技進步能轉化為民眾日常生活的實質改善。


深耕社區第一線:培力在地數位照護專才

「培力民眾成為自己健康的主人」是協會的核心出發點。透過 Google.org 的專案資助,我們將在兩年內培訓 200 位具備數位能力評估糖尿病健康狀況的照護專員 。這些專才將深入全台據點,確保衛教內容「聽得懂、用得上」,並根據不同社區的健康樣態提供客製化的線上互動課程。


數據驅動社群服務:創造 24 萬人次健康互動

計畫預計在兩年內,於全台 300 個受益社區創造超過 24 萬人次的健康互動 。透過健保署以 Google 相關技術開發的數位工具,長者能參與包括肌力訓練、營養衛教等超過 15 種領域的互動課程。此計畫旨在透過數位社群平台,建立完善的轉介機制,當 AI 篩檢發現潛在醫療需求時,能確保高風險個案獲得適當的醫療建議。


數位照護計畫:讓健康台灣落地實踐

數位人道協會創辦人潘人豪表示:「數位人道協會致力於將技術轉譯為社區照護需求。此次獲得 Google.org 的支持,核心是讓創新真正落實於基層,使偏鄉與弱勢族群同樣能享有科技帶來的照護成果。」他進一步指出,這項倡議若全面落地,將透過早期辨識與即時介入糖尿病,有效提升長輩的生活品質,並減輕醫療系統的長期負擔。最終目標,是讓「消弭健康不平等」真正成為可以被實現的台灣未來。

Facing Taiwan’s escalating challenges of diabetes and an aging population, the Digital Humanitarian Association (DHA) has received $1 million in funding from Google.org, Google’s philanthropic arm. Alongside partners, DHA is launching a community-based diabetes care initiative to extend health education and personalized care to 300 grassroots communities and rural locations across Taiwan.


New Digital Care Model: Filling Rural Health Gaps

In rural communities, many seniors lack awareness regarding diabetes complications. DHA emphasizes that to eliminate health inequality, technology must truly enter the community. We are committed to supporting frontline care guidance and behavior change to fundamentally improve chronic disease trends. As a key partner in this initiative, DHA is responsible for bridging the gap in patient education and engagement, ensuring that technological progress translates into tangible improvements in daily life.


Frontline Community Engagement: Empowering Local Digital Care Specialists 

"Empowering people to become masters of their own health" is the core mission of the association. Through Google.org’s funding, we will train 200 care specialists with digital health assessment capabilities for diabetes within two years. These specialists will reach community sites nationwide, ensuring health education is "understandable and usable," and providing customized online interactive courses based on the health needs of different communities.


Data-Driven Community Services: Creating 240,000 Health Interactions 

The project expects to create over 240,000 health interactions in 300 beneficiary communities across Taiwan within two years. Utilizing digital tools based on Google technologies, seniors can participate in interactive courses in over 15 health fields, including strength training and nutrition education. This project aims to use digital community platforms to establish a robust referral mechanism, ensuring high-risk cases receive appropriate medical advice when AI screening identifies potential medical needs.


Digital Care Initiative: Realizing a "Healthy Taiwan" 

DHA founder Pan Jen-hao stated: "DHA is dedicated to translating technology into community care needs. With support from Google.org, our focus is on ensuring innovation truly lands at the grassroots level, allowing rural and vulnerable groups to benefit from the results of technology." He further noted that the full implementation of this initiative will effectively improve the quality of life for seniors through early identification and timely intervention of diabetes, reducing the long-term burden on the medical system. The ultimate goal is to make "eliminating health inequality" a reality for Taiwan’s future.
